Park Chapel operates exclusively for charitable and ministry purposes.  As a participant at a service activity held at the church or in the community, I understand that the purpose of the event is building relationships, helping accomplish partner ministry goals, and connecting with others.

I hereby remise, release, and discharge Park Chapel, in all of its forms and affiliations, and its successors and assigns, from any and all liability, of whatsoever kind and nature, whether known or unknown, foreseen or unforeseen, resulting from the provision of any equipment, products, actions or services. 

I expressly waive in connection with any request for, or provision of, such equipment, products, actions or services, any claims or causes of action, without limitation, that I might otherwise have or obtain against Park Chapel, its pastors, employees, volunteers, agents, fellowship members, related corporations, partners, joint ventures, affiliates, or any provider of such goods and services.

Further, I authorize Park Chapel Christian Church to use photographs and video footage of the event for promotional materials. I have read and agreed to the information given in this entire form, confirmed by checking the box below on behalf of everyone on my registration form.
