Park Chapel operates exclusively for charitable and ministry purposes.  As a church, we work with other local churches, organizations, entities, and individuals to assist in meeting community and individual needs in greater Hancock County, Indiana.  In order to accomplish our purposes, we operate as a clearinghouse of the talents, treasures, and time provided by members of this fellowship.

I hereby remise, release, and discharge Park Chapel, in all of its forms and affiliations, and its successors and assigns, from any and all liability, of whatsoever kind and nature, whether known or unknown, foreseen or unforeseen, resulting from the provision of any equipment, goods, products, or services.  I expressly waive in connection with any request for, or provision of, such equipment, goods, products, or services, any claims or causes of action, without limitation, that I might otherwise have or obtain against Park Chapel, its pastors, employees, volunteers, agents, fellowship members, related corporations, partners, joint ventures, affiliates, or any provider of such goods and services.  

Further, I authorize Park Chapel Christian Church to use photographs and video footage of the participant for promotional materials. My consent and signature is given below. I have read and agree with the information given in this entire form.
